- Description: Provides support and education to teens and young adults as they transition into adulthood and become self-sufficient. The program focuses on delivering rapid response and help directly to the individual at a location most convenient for them. Such as school, home, Juvenile Hall, hospitals, job sites and/or on the street. Services provided include: - Clinical services for mental health improvement and stabilization - Independent living skills including legal emancipation, fiscal management, housing acquisition and maintenance - Health-related supports and services - Education and vocational training services - Employment services - Socialization opportunities
